This is something I posted on another board a few years ago re: 5-fold ministry:
A recent speaker at our church used the term `5-fold ministry of
the
church'. Being unfamiliar with the term, I googled it. The
first
hit was for some seminar promoting it, the second hit was to a site
that claimed the whole idea mostly bunk...
Apparently, the `5-fold ministry' is taken from the following
passage:
"...and *he* has given some apostles, and some prophets, and some
evangelists, and some shepherds and teachers, for the perfecting of
the saints; with a view to [the] work of [the] ministry, with a view
to the edifying of the body of Christ; until we all arrive at the
unity of the faith and of the knowledge of the Son of God, at [the]
full-grown man, at [the] measure of the stature of the fulness of the
Christ;" – Eph. 4:11-13.
apostles, prophets, evangelists, shepherds, teachers. The argument
against the 5-fold ministry, in brief, is that apostles and prophets
are no longer.

I forgot to answer the question in the OP: NO all prophets and apostles are dead.

I believe in Ephesians 4:11 ministries (one might say four-fold.)
I would associate 'missionary' closely with apostle, though. It depends on the kind of missionary.
I don't think every individual church has to have an apostle. Paul was a translocal apostle.
I think we make a mistake if we associate these offices with 'clergy' and just add more offices to clergy. A lot of prophets sit in pews and aren't allowed to funciton because churches have sermon-centered meetings where the preacher does most of the talking instead of the type of meetings we are told to have in I Corinthians 14.
