In regions and areas that has really never had the good news presented to them very much at all? Such as in say a mission outreach into muslim land, or area really into paganism? As would He still operate like in Acts for a reason to get the Gospel and Lord Jesus authenticated, and then go back to the bible only [reaching and teaching once established?

Does God still use Tongues and Sign Gifts today then?

Speaking in tongues meant languages unknown to the speaker but known to the hearers, always in reference to the gospel.
I’ve heard missionaries testify they have seen it happen, so yes, based on that testimony.
As far as other sign gifts (healing for instance), I don’t discount God’s ability or willingness as He sees fit, but I haven’t seen any authenticated healings
